Regarding aeronautical studies for new constructions penetrating the inner horizontal or conical surfaces - does anyone know of a State that has developed logical criteria to limit the number and degree of such penetrations.

I ask because once one dispensation has been granted, it becomes very hard to deny the next application(s).

Note - I assume that regularity is not affected, i.e., there is no increase in OCA/H or procedure minimum altitudes and that safety is the main consideration (think uncontained engine failure on takeoff or manoeuvring in marginal visibility).
