Submission:

One can argue than paragraph 31.2.7.1 refers only to initial and intermediate approach fixes, and therefore is not applicable to STARs. The proposed amendment to PANS-OPS relaxes the satisfactory fix criteria by excluding the cross-track tolerance from 2.0NM limit. That substantially increases the availability of satisfactory fixes for VOR/DME RNAV. Although the amendment is not effective yet, it is very likely to be adopted as proposed.

I consider STARS to be just a convenient way of naming and describing a multitude of initial segments. As far as I can see the intention of an initial segment is to join the enroute phase to the approach phase, which is what a STAR does.)
