Perhaps this is where operator's contingency procedures are involved. On the advice of the Airworthiness Committee, in the 1970s PANS-OPS did include a level acceleration segment after departure and missed approach. However, the operational effect was that departures at Frankfurt were predicated on the Alps and Aukland NZ was practically closed down. This criteria was subsequently removed and replaced by the current text in Vol I relating to contingency procedures, these being the responsibility of the operator.
