ILS-glide path TCH and RDH

I'd like to know about RDH (Reference Datum Height). There has been argument between my fellow and me. That is Y value in calculating TCH on the GP installation. In the FAAO 8260.3B chg18, this is no problem with TCH because we can disregard less than 1.5% of slope between runway centreline and GP with 0 longitudinal slope. However when applying ICAO ANNEX 10, I was wondering if one must really include the "Y" value (GP reflection plan) or not. Is there any exception, where I can calculate without using the Y value as in ANNEX 10?
