The only place I ever had this was in the early days of Cat II in the UK when the restriction to 150 ft was applied to flight director approaches and the 100 ft required use of autopilot. My recollection is that this restriction was later removed. The reason for the restriction should have been stated on the approach chart. I believe that most Cat II approaches are designed for flight director rather than using the smaller autopilot OAS surfaces, possibly because the benefits are not significant at most locations - however, the option is still in PANS-OPS if needed.
